State of California Immunizations To register a child for school,  parents must present the child's written immunization record from a physician, nurse, or health department. Prior to school entry, children must be immunized for:        Polio Diphtheria, Tetanus, and Pertussis Measles, Mumps, Rubella (MMR) Hepatitis B Varicella  A waiver of the immunization requirement may be obtained for religious or health reasons.
